It takes courage to seek out therapy after an injury. Our team at Therapeutic Rehabilitation Centers is here to make the admission process as simple as possible for our clients, their families, and referral sources. Our goal is to empower our clients with hope and determination, to forge ahead in maximizing their ability to perform daily activities in a way that enhances their quality of life. Welcome to the first step on your recovery journey.

Make a Referral

At the bottom of the page, you will find a shortened version of our online referral form for your convenience; the form will take no more than three minutes to complete. Once our Admissions team receives the information, we will reach out within twenty-four hours to schedule a consultation.

We also provide a longer, complete version of our referral form available online. Please be aware this form takes between 15-30 minutes to complete. You can find the link to that form here: New Patient Referral Form

To fill out the New Patient Referral Form, you will need to provide:

• Demographic information (name, address, date of birth, phone number, etc.)
• Care team information (case manager, vocational specialist, prescribing doctor, attorney, guardian, emergency contacts)
• Prescription(s) and evaluations (therapy prescriptions, neuropsych evaluation, behavioral concerns)
• History of prior therapy, if any
• Current list of medications
• Transportation method
• Employment and education history
• Insurance information
